SCOPE: This position serves as a source of general information and guidance for employees, including direct care workers and office staff on issues affecting their ability to maintain employment. This position also provides individual case management to direct care staff identified as having an elevated risk of quitting or having attendance or performance problems due to life issues outside of work.

Job duties include:

  • Plan, Schedule, organize, and conduct classes on life skills, including personal planning, personal budgeting and money management, nutrition and meal preparation on a budget, goal setting, job readiness, values and beliefs that support career growth, conflict resolution, anger and stress management, substance abuse, domestic violence sexual abuse, housing an combatting homelessness. Classes will be combination of in-person, live virtual, and pre-recorded.
  • Classes on other information of interest to DCWs, with focus on building safety net and life security.
  • Research and write social media posts, website blog posts, e-newsletter articles, success stories, and case studies. Record video blogs, interview blogs with subject matter experts.
  • Information and referral for individuals in specific need.
  • Demonstrate the ability to research and network with available resources.
  • Targeted case management for individuals identified as at risk of calling off work and/or quitting.
  • Refer DCWs to appropriate services when issues go beyond the scope of this position.
  • Serve as role model for at-risk DCWs.
  • Maintain case notes, goal plans, progress notes on DCWs identified as in need of targeted case management.

Skills:

  • Excellent problem-solving, interpersonal, organizational, and communication skills required
  • Ability to effectively handle stressful and emergency situations with tact, professionalism, a clam disposition, and diplomacy, required.
  • Ability to be sensitive to and accommodating of a variety of life choices and cultural backgrounds, required.
  • Hands-on experience with various computer programs including HCSIS, Zoom/Turbo Meeting, and Microsoft Office products.
  • Written, telephone, typed, and in-person communications, including public speaking, skills required.
  • Ability to maintain detailed case notes on a timely basis within 3 days from contact.
  • Ability to travel as necessary throughout CRI service area with appropriate breaks. Overnight travel may be required on occasion.

Qualifications:

  • Bachelor’s degree in sociology, social work, psychology, gerontology, criminal justice or other related social sciences.
  • Or a combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years professional experience in developmental disabilities, special education, mental health, counseling phycology, social work, or health and rehabilitative services.
  • Work experience in home care, home health care, or other long term services and supports for the elderly, people with physical disabilities, and/or intellectual disabilities preferred. Strong preference for direct care/direct support experience.
  • Must have current and valid driver’s license, a reliable vehicle with current and valid car insurance.
  • Must be able to pass various background checks.
  • Ability to be covered under CRI liability insurance.
  • Ability to work outside of the regular work schedule on occasional basis as needed.
  • Other times may be required for business travel, trainings, and meetings, as necessary.
  • Crisis situations may require additional unplanned availability.
