What you do:

The Resident Care Coordinator directs the health concerns of each Assisted Living Resident including resident assessments, medication/pharmacy management and resident activities of daily living and personal care needs. Hires and supervises care staff for assisted living unit. Directs and ensures that services are performed in consideration of the health, welfare, safety, and the satisfaction of our Residents.

• Collaborates with the Executive Director on day-to-day operation of clinical services for the entire facility.
• Ensures that each resident’s pre-admission screen is completed prior to move-in. Monitors routinely for changes in condition to ensure changes are addressed.
• Completes level of care assessments of Residents to determine the initial and ongoing needs of the residents’ medical and personal care requirements.
• Ensure that adequate staffing and schedules are posted accordingly to meet staffing requirements set by Union Contracts and State guidelines.
• 
  Coordinates all pharmacy/medication needs and ensures compliance with applicable state rules and regulations, and company policy and procedures.
• 
  Assists with, and provides as needed, in-services and trainings for staff, such as medication management, resident care, abuse and neglect, etc.
• 
  Assists the Executive Director/designee and the sales and marketing team in business development plans and goals.

What you need:

• 
  LPN with a degree from an accredited school of nursing or a social work Degree. Preferred, but not required.
• 2+ years of management experience, preferably in the Healthcare field
• Knowledge of State Rule and Regulations pertaining to operating an Assisted Living Facility.
