We are seeking a Certified Occupational Therapy Assistant to join our team!!

This position is Part-Time

Job Responsibilities:

  • Travels to patient homes and provides occupational therapy care and patient assessment duties. Implements treatment plan. Conducts treatments for exercise, posture correction, etc. Observes, records, and reports the patient’s reaction to treatment and changes in condition. -- Instructs patients, family members of patients and other health personnel in the use of equipment, or phases of therapy with which they may work with patient
  • Ensures for appropriate discussion of patients and required completion of documentation. Assist in the implementation of vocational and educational programs to restore, reinforce and enhance task performances, diminish or correct pathology and to promote health and self-sufficiency.
  • Design and adapt equipment for working and living environment. Fabricate devices to assist and improve function, independence and participation in the program.
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve and restore strength, coordination, range of motion and function.
  • Coordinates discharge plan decisions and the final assessment with the Occupational Therapist.
  • Coordinates on-going care with Occupational therapist, the RN Case Managers and other clinical staff to ensure coordination of care. Reports any concerns or issues. Identifies any barriers to achievement of identified goals/outcomes and works in collaboration with the RN Case Manager and other patient care team members to assure coordination of care is provided to attain desired outcome..
  • Consults with therapist, nurses, social workers, and other health care workers; contributing to patient care conferences.
  • Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.
  • Documents patient care services by charting in patient and department records.
  • Maintains patient confidence and protects hospital operations by keeping information confidential.
  • Communicates significant changes in patient condition.
  • Documents plan, notes, requirements per regulations in timely manner and accurate.
  • Follows-up with OT or RN Clinical Manager to ensure notification and documentation of missed visits is provided in accordance with proper procedures in a timely manner.

Education Requirements

  • Degree from an accredited Occupational Therapist Assistant Program
  • Electronic medical record charting and data entry.
  • A minimum of six months occupational therapist assistant experience preferred with home health
  • Current CPR Certification
  • Current and unrestricted COTA licensure eligible for Wisconsin

Position Requirements

  • Requires travel to patient homes and meeting locations.
  • Must maintain a valid Wisconsin driver’s license.
  • Must maintain an acceptable driver’s record .
  • Must maintain proof of current auto insurance.
  • Must have access to a vehicle in good working condition.

Job Type: Part-time

Pay: $27.00 - $30.00 per hour
