Job description

We are looking for an LVN to assist the RN in directing and coordinating nursing services for persons with intellectual disabilities in a HCS (Home and Community Based Services) setting, in accordance with HCS principles at McKinney Direct Care Services (MDCS, LLC. The LVN will report to the RN.

Essential job responsibilities are as follows:

• Instructs, evaluates, and performs delegation of nursing duties to unlicensed assistive personnel (direct care staff, day hab, etc).
• Lead and train the direct care team to manage clients’ medication schedule and monitor and observe clients’ medication intake.
• Responsible for an awareness of client conditions and problems and for maintaining an acceptable level of nursing care within all the programs.
• Serves as responsible person in delivery of medical services and nursing evaluations including comprehensive nursing assessments.
• Coordinate medication supplies and appointments.
• Responsible for medical emergency management.
• Conduct over the phone triage.
• Ensures that all medical consultations and laboratory reports are documented in the clients’ records and results communicated to the family in a timely manner
• Update clients’ MARs as prescribed by her physicians, perform assessment reports, and participate in meetings on establishing plan of care for clients.
• Monitor the delivery of medical related needs training, medications, and side effects to ensure quality of services is delivered in a timely manner.
• Ensure state and HCS guidelines are being followed through the delivery of nursing services.
• Ensure all clients medication management documents are placed in a secure location.
• Complete required documentation as outlined with the policy and procedures of MDCS and the Texas HHSC IDD Services ICF-ID and TxHmL and HCS waiver programs and guidelines.
• Cooperate with audits, inspections, and investigations
• Responsible for preparedness for all audits; complete and submit monthly quarterly reports.
• Perform other duties as assigned by supervisor(s).

License/Certification:

• LVN License
• Driver's License
• CPR

Job Type: Contract

Pay: $20.00 - $29.00 per hour
