Job Status: Full Time

Work Schedule: M – F 8a – 5p & alternating weeks on call

Amount of Travel Required: 30%

Positions Supervised: Field RNs/LPNs

POSITION SUMMARY

Manage patient health problems and needs, develop and implement a plan of care, team schedules, and maintain medical records. Utilize team to administer and carry out Plan of Care. Advise team on health maintenance and disease prevention and provide case management. Licensing without restrictions required.

Essential Functions

● Manage patient care, staff, and scheduling.

● Maintain accurate, detailed reports and records.

● Review referrals, admit and discharge patients as needed.

● Work with recruiter on staffing needs including performing interviews with candidates.

● Complete post-hospital visits, recertifications, and summaries every 60 days for patients.

● Manage medication orders and classifications and make sure the medication assessment record is up to date.

● Ensure education for patient’s family is completed.

● Ensure nursing education and supervision is maintained.

● Read and upload notes to ensure patient care and needs are accurate.

● Process, validate, and analysis of OASIS.

● Create 485 from OASIS assessment.

● Modify patient treatment plans as indicated by physician orders and patients' responses and conditions.

● Direct and supervise skilled nursing or healthcare personnel or supervise a particular team.

● Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.

● Monitor all aspects of patient care, including POC being carried out as written and goals met.

● Instruct individuals, families and other groups on topics such as health education, disease prevention, and develop health improvement programs.

● Ensure team is educated on the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems.

● Inform physician of patient's condition as required.

● Observe nurses and visit patients to ensure proper nursing care.

● Professionally manage patient, staff, and doctor issues as needed.

● Act as a liaison between the doctor and staff.

● Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions.

● Recommend where appropriate drugs, medical devices or other forms of treatment, such as physical therapy, inhalation therapy, or related therapeutic procedures.

● Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing.

● Engage in training or research activities related to nursing.

● Perform other duties as assigned by management.

EDUCATION/EXPERIENCE AND/OR TRAINING

Education:Registered Nursing Program

Experience:One to two years related experience

· Valid Louisiana RN license without restrictions

· CPR - BLS

OTHER QUALIFICATIONS

  • Has not been excluded from participating in Medicare and Medicaid programs.
  • Maintains eligibility to participate in Medicare and Medicaid programs throughout employment
  • Capable of providing emergency care as needed for any patient
  • Maintains a neat, well-groomed appearance at all times. Keeps personal appearance appropriate for patient care.
  • Ability and willingness to participate appropriately in a disaster or evacuation situation.
  • Participate in pre-employment and annual Tuberculosis screening.

Requires full range of body motion including:

  • Standing/walking: Frequently. Worker will be spending considerable time up and about in the facility doing various tasks per job description.
  • Bending/Stooping: Occasionally. Worker will be bending or stooping to work with supplies, preparation of documents, filing, working with clinical records, etc.
  • Lifting/Handling: Occasionally/ Worker will be lifting/handling supplies to do inventory, restock, ordering, as well as filing and auditing of clinical records, etc. Range of weight: 1-50 pounds.
  • Carrying: Occasionally. Worker will be carrying clinical records, manuals and supplies, etc. Range of weight: 1-50 pounds.
  • Pushing/Pulling. Occasionally. Worker will be pushing/pulling on various supplies storing, moving and taking inventory of supplies as well as with clinical records filing, auditing, preparing for each month, etc. Range of weight: 1-50 pounds.
  • Balancing: Frequently: Worker will be balancing supplies to be stored, moved from storage to using area, etc. When moving records from active to inactive status or during auditing periods, worker will be balancing legal documents that are not secure.
  • Pivoting/Turning: Frequently. Worker will be pivoting and turning to stock supplies, handle records, etc.
  • Crouching/Stooping: Frequently. Worker will be crouching/stooping to file records, working with supplies to restock inventory of supplies, etc.
  • Kneeling: Occasionally. Worker will be kneeling to file records, stock and inventory supplies, etc.
  • Reaching: Occasionally. Filing administrative folders, records; nursing file folders.

Primary office and occasional patient home visits.
