Community Integration Specialist - Residential Transitional Services Team
REPORTS TO: Program Manager
DEPARTMENT: Community Housing Support Teams
HOURS: 35 Hours per week
QUALIFICATIONS:
BA in Human Services with three years’ experience working with persons with psychiatric disabilities. Masters Preferred.Clean valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
JOB FUNCTION:
Oversee services regarding community integration, linkages to community resources and ADL training for individuals with psychiatric disabilities in a scattered site housing program or working with clients preparing to transition into independent housing.
Job Type: Full-time
Pay: $40,000.00 - $45,000.00 per year
