I am working with a Health Care Center in 3 different locations in
Indiana. We are looking for a
Certified Nurse Midwife to join us
full-time in
Hammond, Portage, or Chesterton!
We are a non-profit organization dedicated to providing quality, affordable healthcare to everyone, including the uninsured and underinsured. Patients are seen on both a scheduled and walk-in basis.
The Certified Nurse Midwife (CNM) is expected to be familiar with the philosophy, goals, and objectives of the PCMH model, and work collaboratively with other members of the patient care team to achieve maximal health outcomes for our patients. Under the supervision of collaborating physicians, you will assume multiple clinical functions involved with the care of patients, that fall under your licensure and training. The services include diagnosis, treatment, and coordination of care, for preventive and health maintenance to female patients of all ages. You must meet and maintain credentialing guidelines, as well as in affiliated hospitals in Lake and Porter County, Indiana, as applicable, and must demonstrate clinical competency as a Certified Nurse Midwife.
Essential Job Functions:
Assessment of the health status of patients, formulation of diagnosis and implement appropriate treatment plan designed to maximize the health of patients, maintaining a process for systematic evaluation of patient status, seeking to promote maximal health among clinic patients, providing educational opportunities for each patient, participate in peer reviews, prepare reports and documentation, attend meetings and training sessions as required, along with other duties as assigned.
Qualifications:
- Graduate of an approved master’s Nursing Program for Nurse Midwifery that fulfills the educational requirement for licensure as an Advanced Practice Nurse, and Certified Nurse Midwife in the State of Indiana.
- Must be board certified as a Nurse Midwife
- Be in good standing with National Practitioner Data Bank
Licensure:
- Valid RN license.
- Valid APN License
- Valid Nurse Midwifery License
- Valid CRS Certification
- Valid Federal DEA Licensing
- Meets Credentialing guidelines of NSHC and with local hospitals.
