Lake Park Residential Care (Lake Park), a residential care and assisted living facility, provides high-quality, long-term care for homeless and low-income adults coping with complex and severe mental health issues. Our highly-trained and compassionate mental health professionals offer Lake Park residents structured and sustained support. Lake Park Residential Care is a member of the Regional Care Group.

Position Purpose:

To provide case management and ADL services to SMI, CA and SED clients and their families. These services are directed toward: providing the skills and supports necessary to enable clients to function in the least restrictive environment with the highest possible level of functioning. Case managers provide assessment, service planning, coordination and linkage and monitoring functions.

Essential Functions:

• Establish appropriate working relationship with client (s) and conduct interview during which necessary information is gathered.
• Make recommendations which are appropriate given the clients strengths, weaknesses, and unique circumstances and characteristics including issues of diversity
• Construct attainable, and measurable behavioral treatment objectives based on the client’s and/or the client’s family’s abilities, desires and resources
• Review treatment on a periodic basis and modify treatment planning according to clients progress or lack of progress
• Ensure that client has sufficient meds as prescribed by the doctor
• Perform drug screens as requested by psychiatrist/and or therapist.
• Document visit/attempt at visit with clients discharged form inpatient care within 48 business hours of that discharge
• Effectively de-escalate potentially violent or out-of-control situations through active listening and limit-setting skills

Schedule:

Days, Tuesday – Saturday

Requirement:

• BA/BS in psychology, social work or related clinical field
• 1 year experience with target population.

Job Type: Full-time

Pay: $17.00 - $22.10 per hour
