Case Manager/Care Coordinator

EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 54,000 people in need through 100 programs across Long Island and NYC, seeks a Case Manager/Care Coordinator o work full time for its Offender Reentry program.

The Case Manager/Care Coordinator is a non-exempt position paying per hour $21-26 at 35 hours per week. Monday-Friday 9:00AM-5:00PM. The office is located in Brooklyn, NY and role reports to Program Director.

Primary Purpose of Job:

The ORP Case Manager/Care Coordinator will assist clients with co-occurring substance use and mental disorders (COD) returning from state prison to NYC. The Case Manager is responsible for outreach and utilizing CTI/RNR framework, including, referral, linkage; assessment, reassessments and transition planning.

Principal Duties & Responsibilities:

  • Administer assessment tools at DOCCS facilities, via video conference and in the community.
  • Conduct a benefits eligibility screening and assist clients in obtaining benefits as indicated, including public assistance, SSD/SSI, housing, medical and other needs.
  • Develop integrated reentry service plans.
  • Arrange for and implement placement into appropriate services.
  • Provide ICM including: skills building, boundary spanning, counseling, advocacy, interactive journaling, CBT, and family centered interventions.
  • Maintain complete client records and have available for inspection by Supervision at any time.
  • Escort clients to appointments.
  • Make visits to housing and treatment facilities to ensure compliance with treatment plans.
  • Conduct drug testing on clients of the Program.
  • Maintain caseload through ongoing client contact.
  • Attend meetings and clinical trainings.

Knowledge, Skills & Abilities Required:

  • Bachelors Degree in a relevant field of study and at least one year of experience working with those who are involved with the criminal justice system preferably those with clients with co-occurring substance use and mental disorders.
  • Knowledge of available treatment services and ability to work cooperatively with housing, treatment and mental health systems.
  • Must have valid New York State Drivers License.
  • Good communication skills, both oral and written.

This position requires the incumbent to provide acceptable proof of COVID-19 vaccination
