Care Manager Responsibilities include but not limited to:

• Ensure company standards as a high-quality care provider through the implementation of exceptional customer service and outstanding positivity.
• Generate positive reviews on Google from caregivers and clients.
• Call each active caregiver (during the caregiver’s non-working hours) monthly to follow up with client and office satisfaction.
• Report on the provided spreadsheet the status of each caregiver satisfaction review.
• Conduct new caregiver staff orientation.
• Conduct satisfaction reviews, revise care plan and update any needed paperwork with every active client every quarter per Indiana’s State Guidelines.
• Document each quarterly review on the provided spreadsheet.
• Conduct check-in visit with new caregivers 2 times in their first month of employment.
• Call new caregiver(s) weekly until she/he completes first 60 days.
• Recognize and reward good job performance and promote caregiver with positive recognition.
• Generate one caregiver participation contest per month.
• Generate one “Angel of the Month” award.
• Conduct home visit for potential clients when needed as a backup to Marketing Director.
• Deliver thoughtful birthday gifts for caregivers and clients.
• Take pictures of caregiver interaction with clients/pictures on their birthday given they have provided release of image documentation.
• Maintain absolute confidentiality of all information pertaining to employees, clients and client’s families.
• Maintain positive relationships with all clients, caregivers, and referral sources.
• Independently respond to and resolve any clients’ complaints or concerns and inform office as they occur.
• Communicate and refer appropriate matters to the Office Manager for direction.
• Perform other functions as deemed appropriate by the management team.
• Maintain Care Manager phone at home.

A successful applicant will meet the following requirements:

• Associates Degree is preferred with a minimum of two years of experience in an office setting, preferably health or home care.
• Demonstrate proficiency in Microsoft Office (Word, Excel and Outlook) applications, scheduling systems and other health care industry related software.
• Ability to listen and communicate clearly, fluently and diplomatically orally and in writing.
• Ability to remain flexible, resilient, calm and maintain a sense of humor; and present a well-groomed professional image.
• Ability to be timely and early to all scheduled appointments.
• Ability to plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines, as well as work independently with a minimum amount of direction and/or supervision.
• Ability to generate goodwill for the agency with clients, their family members, and referral sources. Demonstrate a strong commitment to client service excellence.
• Possess and maintain good physical and mental health, including current TB testing.
