Job Summary and Qualifications

The Behavioral Health Advanced Clinical Pharmacist is responsible for providing general clinical and pharmacy services in compliance with regulatory and hospital policies/procedures. The Pharmacist will provide patient care through the use of knowledge, skills and abilities in compounding and dispensing pharmaceuticals. They are responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in the hospital’s solid organ transplant units. They use advanced knowledge, skills, and professional judgment in all aspects of their work.

What you will do in this role:

  • You will interpret orders and transcribe/verify in the pharmacy computer system efficiently.
  • You will accurately compound and dispense pharmaceuticals.
  • You will monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
  • You will consult with Physicians and nursing staff members concerning the availability, dosage form, side effects, and possible incompatibility of new as well as currently used drugs. You will suggest appropriate, cost-effective therapeutic medication alternatives to medical staff.
  • You will sustain the hospital formulary, minimizing nonformulary procurements, utilizing therapeutic interchanges and promoting rational drug therapy selection, limiting unnecessary procurement costs.
  • You will provide accurate and timely medication information to healthcare staff and patients.
  • You will maintain appropriate controlled substance records.

  • You will assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.

What qualifications you may need:

B.S. or Pharm D. degree in Pharmacy required.

Graduate of an accredited PGY-2 residency program required in Behavioral Health.

Five years’ inpatient hospital experience.

Pharmacist Board Certification – Psychiatric Pharmacist (BCPP) within one year of hire (or Pharmacotherapy with multiple years of experience in the behavioral health service line.)

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).

Live Oak Mental Health and Wellness Center, is a part of Trident Health. We give inpatient stabilization treatment to patients who seek treatment for acute psychiatric symptoms which cannot be addressed on an outpatient basis. Common disorders of patients treated in this program include but are not limited to: major depressive disorder, schizoaffective disorder, schizophrenia, bipolar disorder, adjustment disorders, post-traumatic stress disorder, impulse control disorder and a variety of co-morbid personality disorders. Our patient population is both adult and adolescent. We offer 24 hour availability of a full range of services. We have the capability to institute rapid lifesaving interventions. Our physician availability is 24 hours a day to institute needed modification to the treatment plan. Psychiatrist involved in the direction and management of the treatment program. Round the clock skilled nursing care implementing the treatment plan and providing monitoring and assessment of the patient’s condition and response to treatment.
