The Assistant Coordinator functions under the supervision of the Falls Prevention Coordinator to provide participants with exercises, activities and education to help seniors decrease their risk of falling and improve their overall health. The Falls Prevention team works closely with the Center for Aging Resources and Enrichment (CARE) leadership team to participate in community events, weekly programming, and other grant opportunities as they become available. The Falls Prevention team also works with students and their professors from the Purcell School of Professional Studies at Le Moyne College to help implement the Falls Prevention Program. This position offers the opportunity to work closely with older adults in the community and make a visible difference in their lives. This is a grant funded position that is currently funded up to May 31, 2023.



PRINCIPAL DUTIES AND ESSENTIAL FUNCTIONS

  • Under the direction of the Falls Prevention Coordinator assists in evaluation and development of the rehabilitative plan of care and in periodic re-evaluation as required.
  • Participates in the preparation of clinical and daily progress notes. All notes are reviewed by the Falls Prevention Coordinator. Also prepares periodic summary reports.
  • Together with the Falls Prevention Coordinator, instructs the participates and assists in teaching them exercise programs.
  • Guides and instructs the patient in prescribed therapeutic self-care and creative activities that are directed toward improving independence and physical and mental functioning and assists the participants in the use of self-help devices.
  • Observes, and records activities and findings and reports to the Falls Prevention Coordinator, the participants reaction to the program and any changes in the participants condition.
  • Consults with Falls Prevention Coordinator regarding the outcome of home visits and reports physical findings in written form Job expectations for all Le Moyne College employees:

  • Engages in a planned program of professional development. Continually maintains skills and knowledge relative to the position and technology industry including best practice, evolving operational requirements, and industry trends.
  • Performs a variety of related duties as assigned.

Job Requirements:
  • Excellent organizational, project management, and computer skills
  • Ability to treat each client, volunteer, and staff member with dignity and respect
  • Sensitivity, flexibility and top-of-the-line interpersonal skills
  • Enthusiasm for the mission of the organization and the goals of the program
  • Must have excellent English speaking and writing ability Education/Experience:
  • Graduate degree in either Physical Therapy or Occupational Therapy preferred
  • Current license in NYS.
  • Experience as a Fitness Instructor, preferred but not required
  • Must have a reliable vehicle
  • Experience providing home visits and therapy in Onondaga County. (Mileage reimbursed)
