GENERAL SUMMARY/ OVERVIEW STATEMENT:


Provide patient care at Spaulding Cape Cod according to the philosophy and standards of Massachusetts General Hospital and Spaulding Rehabilitation Associates. Execute and evaluate treatment techniques within the scope of an Advance Practice Provider (APP) to assess and manage the program population; demonstrate advanced problem solving skills in all aspects of medical and patient assessment, care and treatment. Use appropriate clinical skills to integrate and implement the clinical management process with the plan of care. Function as a clinical resource for the patient, consistently providing direction and support to ensure a focus in the team goals and patient driven outcomes. Function within role expectations, delegate duties to other caregivers as required. Serve as a resource to members of the care team and office staff.


PRINCIPAL DUTIES AND RESPONSIBILITIES:

  • Holistic, comprehensive patient care is provided in alignment with medical management team and in accordance with the clinical process and practice standards.
  • Complete medical database is assessed including health history and physical examination upon admission of selected patients.
  • Medical conditions are diagnosed, medical consultation is planned and ordered, and interventions are based on assessment and protocols under the direction a supervising physician or designee.
  • Patient’s response to disease process and therapeutic interventions is evaluated and plans revised to achieve optimal patient outcomes.
  • Plan of care is developed and implemented that reflects the needs and goals of the patient. Serve as an advocate for the patient.
  • Seek recommendations and guidance from supervising physician and leadership as needed.
  • Documentation includes all assessment data, orders, interventions, results and evaluations in the patient medical record per established standards.
  • Information, education and support are provided to patients and families, including them in care and discharge planning.
  • Quality improvement programs are participated in to achieve established goals.
  • Clinical care activities are coordinated in collaboration with other care team members to facilitate achievement of patient goals.
  • Ancillary clinical staff are coordinated and supported to achieve established performance goals.
  • Clinical assessment is completed accurately and timely in accordance with practice standards.
  • Medications are administered safely and according to professional standards of practice and clinical policies and procedures.
  • Complete administrative processes, trainings, meetings and other needed activities within role.
  • Ensure all work is performed within the scope of license.
  • Respond appropriately in emergent situations; engage in life saving efforts as needed.
  • Utilize, troubleshoot and maintain all necessary equipment used within the position.
  • Additional department, organization, or network activities are completed per established objectives.
  • Follow all organizational policies, including HIPAA compliance as well as organizational values of Innovation, Collaboration, Accountability, Respect, and Excellence are upheld.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED


Advanced understanding of the application of clinical process and care to patients in a rehabilitation setting


Demonstrated competence in specialized area.


Ability to properly assess and execute patient care and modify practice as directed.


Knowledge of medication management and applicable regulations.


Computer proficiency required, especially in the updating of the patient record; accurate data entry and documentation skills


High level of service delivery with ability to work with diverse populations; able to effectively communicate clinical and/or technical information to patients, family members and staff.


Ability to build relationships and collaborate effectively with a diverse population. Listen, understand and respond to concerns of patients and families, while displaying compassion and advocacy for patient needs.


Comply with all organizational policies, procedures and instructions, including HIPAA and maintain confidential and sensitive information.


Maintain variable work schedule to meet department needs (evenings, holidays and weekends).


Ability to work independently, be self-directed and contribute as a member of the team.


LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable):


Current licensure in the Commonwealth of Massachusetts as an advance practice provider, either as a nurse practitioner or physician assistant


Current certification in ANP/FNP or PA


ACLS required


EDUCATION:


Master’s Degree in Nursing or Physician Assisting required


EXPERIENCE:


Preferred 3 years of direct patient care experience in an advanced practice level with at least one year being in an acute setting

FISCAL RESPONSIBILITY:


Prudent use of hospital resources


WORKING CONDITIONS:


Inpatient units in an inpatient rehabilitation hospital. Periods of prolonged, standing, sitting and other physical requirements occur. Potential exposure to workplace hazards such as bodily fluids and sharp instruments.
