Be part of the Saint Therese difference! We are seeking a Admissions Coordinator (RN/LPN) for one of Minnesota's leading nonprofit senior housing and service providers.

The position is full-time working 8:00 a.m. - 4:30 p.m. Monday through Friday. This position will participate in a Manager on Duty weekend rotation.

Saint Therese offers a continuum of care and services available in a person's own home or in one of our four Twin Cities communities: New Hope, Brooklyn Park, Shoreview, and Woodbury.

Rooted in history, Saint Therese of New Hope is our largest community. This care center has 168 skilled nursing beds for long-term and transitional care. In addition, the Residence at Saint Therese of New Hope houses 224 independent apartments and suites for assisted living and memory care.

Responsibilities: Evaluates/assess documents, directs and provides nursing care. Communicates status changes to family members or responsible party. Informs house supervisor and medical provider of changes or needs. Performs all duties consistent with Quality Standards and infection Control practices. Performs all job functions within State and Federal Regulations.

Qualifications:

Education: Successful completion of an approved program for an Licensed Practical Nurse or Registered Nurse.

Experience: Minimum three years long-term care experience preferred. Experience in clinical assessments and evaluating care needs, preparation of care plans, insurance authorizations and Medicare/Managed Care guidelines.

Special Knowledge, skills, and abilities:

  • Demonstrated knowledge of State and Federal regulations relating to clinical assessments, MDS and care plans.
  • Demonstrated extensive knowledge of Medicare, HMOs, and Managed Care coverage guidelines.
  • Demonstrated ability to communicate with and utilize outside resources and agencies for questions related to coverage and admission criteria for Medicare, Managed Care, Long Term Care and other insurances.
  • Good written and verbal communication skills.
  • Time management and prioritization skills.
  • Ability to collaborate with the clinical team and other departments during the admissions process.
  • Demonstrate proficiency in reviewing referrals from hospitals, clinics and the community in order to accurately assess the clinical needs of potential admissions.
  • Demonstrated leadership and management abilities.
  • Demonstrated knowledge of computers and related software and effective use of systems in place.
  • Ability to work with the clinical team to identify potential quality of care issues at time of admission and present solutions or care alternatives.
