Hours/Schedule:

• Part-Time: 8 days/pay 4:00PM - 8:00PM (*every other weekend/every other holiday required)
• Full-Time: 80 hours/pay 9:00AM - 5:30PM (*every other weekend/every other holiday required)

Responsibilities:

• Assist in developing and maintaining an activity schedule for residents that includes various meaningful activities
• Assure needed supplies are available and used in an efficient manner
• Coordinate activities with various departments
• Participate in discharge planning, development and implementation of activity care plans and resident assessments
• Assure all activity progress notes are charted, informative, and descriptive and include resident's response (Includes initial activity assessment, care plan, progress summary, admissions, readmissions, discharges, interdisciplinary notes, and activities log
• Operate facility vehicles in order to transport residents to various functions and outings (clear driving record and proof of insurance/financial responsibility required)
• Encourage residents to participate in activities
• Supervise and teach activities to residents on an individual or group basis
• Prepare monthly activities calendar and newsletter
• Assist in developing preliminary and comprehensive assessments of the activity needs and goals of each resident / review and revise the plan of care as needed/scheduled
• May arrange for sale of articles made by residents

Educational/Experience Requirements:

• High School diploma or equivalent (required)
• Current, valid, unencumbered driver's license (Issued by the State of Ohio Bureau of Motor Vehicles (required)
• Previous experience in therapeutic recreation or related field and/or long-term care preferred but not required

Physical Requirements:

• Must be able to sit, stand, bend, lift and move intermittently during working hours

Perfect Attendance Bonus: Quarterly

Pay: $12.20 per hour
