POSITION SUMMARY

Under the direction of the YCES Regional Coordinator, the YCES Matcher will match youth (ages 18‐24) experiencing homelessness and housing instability, who have been identified through the Coordinated Entry System for Youth (CESY), to short‐ and long‐ term housing resources as they become available in SPA 6. The YCES Matcher is responsible for “matching” youth to housing using the Homeless Management

Information System (HMIS) and CES policies and protocols that prioritize individuals who are most appropriate for the centralized housing resources. This includes working with other local homeless service providers, outreach teams, PSH providers, the Department of Mental Health, the Department of Health Services, City of Los Angeles

Housing Authority, and the County of Los Angeles Housing Authority to ensure different housing eligibility criteria are met. The YCES Matcher will track and manage the matching process through HMIS and other CES tracking systems.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Actively manage the community queue to assess and determine eligibility for housing resources
  • Utilize the Coordinated Entry System (CES) database (i.e., HMIS) to match individuals to appropriate housing options
  • Notify housing navigator or assigned case manager that housing option has been identified
  • Follow up and schedule meetings with applicants and referrals in a timely manner
  • Communicate referral updates to referring partners and organizations, clearly provide information on permanent housing opportunities to partner agencies.
  • Communicate referral needs of youth who do not otherwise qualify for subsidized housing resources and programs.
  • Maintain accurate and well‐organized records, including eligibility documentation, and all incoming, pending, and closed referrals
  • Track progress and outcome of matches, and update HMIS accordingly
  • Communicate referral updates to referring partners and organizations
  • Partner on and participate in outreach efforts to youth experiencing homelessness in South Los Angeles
  • Support regional care coordination meetings in partnership with the YCES Regional Coordinator
  • Facilitate the YCES case conferencing meetings to capture client’s housing progress towards permanent housing
  • Represent CRCD and SPA 6 in Youth/CES‐related meetings and trainings and be prepared to provide the most updated information to the community
  • Assist the YCES Coordinator to schedule presentation for the care coordination meetings
  • Keep an up‐to‐date tracking sheet with all youth providers in SPA6. Track the services and resources each agency provides for youth and update the HSSD of any changes
  • Create and update a contact list for all providers in SPA6 and keep up to date contact information such as email address and phone numbers to be included in YCES Coordination and case conferencing meetings
  • Facilitate partnership with community resources
  • Meeting with stablished partners to review, evaluate and support housing plans as needed
  • Organize and providing training to community partners on matching policies, LAHSA guidance, and subsidized housing applications
  • Ensure housing program data is accurate and documented appropriately
  • Attend and complete staff development training as assigned
  • Provide assistance at CRCD’s drop‐in center for homeless youth, including, but not limited to, being present at the front desk, answering phones, checking and returning messages, and maintaining client files

MINIMUM QUALIFICATIONS

  • 1‐2 years’ experience or related work and a Bachelors‐level Degree; OR an Associates‐level Degree and 5 or more years’ experience working with young people facing multiple barriers to housing stability.
  • 2 years’ experience working with HMIS or similar database desired.
  • Knowledge of the Coordinate Entry System (CES), South Los Angeles community resources and social service providers desired.
  • Experience working with youth experiencing homelessness and housing instability, and/or have had experience in foster care, probation, mental health systems preferred.
  • Background in Mental Health services, Trauma Informed Care, and/or Harm Reduction practices desired.
  • Reliable, flexible team player who works well with minimal supervision, who has a good work ethic, and an ability to set and maintain personal boundaries.
  • Demonstrated ability to work on multiple projects simultaneously, and to meet deadlines.
  • Reliable transportation: valid driverʹs license, and car insurance as required by law, as appropriate.
  • Proficient in Microsoft Office Suite

WORK ENVIRONMENT

  • On occasion walk or drive to different local sites throughout the day.
  • Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 lbs.
  • Combination of field and office environment.
  • May necessitate working in busy and loud environments.
  • May be exposed to elements like cold, heat, dust, noise, and odor.
  • Work regularly in a community that is exposed to gang activity, substance use, homelessness, and high unemployment rates.
  • This is not a work‐from‐home position.
