Provides direct care to residents, while promoting the resident’s highest level of functioning, as directed by management. Assists in maintaining the resident’s environment in a safe and neat manner. Promotes quality services within community standards, State and Federal Regulations.

Duties and responsibilities

  • Manages the health care of all residents, including the dissemination of information to staff.
  • Performs on-site assessments of residents admitted to hospitals or skilled nursing facilities.
  • Completes a pre-admission assessment for each resident
  • Initiates a resident service plan according to the individualized needs of the resident, as prescribed by physician and/or residence policy.
  • Performs ongoing assessments of residents' physical and psycho-social needs and coordinates with other departments to ensure quality, proactive care.
  • Conducts pre-admission screening of prospective new residents. Ensures all required documentation is completed prior to or upon resident admission, including nursing assessments, service plans, and other assigned forms.
  • Observes residents; records significant conditions and reactions, and notifies resident care manager and physician of resident's condition and reaction to drugs, treatments, and significant incidents.
  • As appropriate, explains procedures and treatments to residents to gain cooperation, understanding, and allay apprehension.
  • Communicates with resident's physicians regarding residents' status and condition changes.
  • Facilitates continuity of care for those residents receiving home health care services.
  • Provides training and education to resident care staff on an ongoing basis including classroom in-services and situation-specific training.
  • Ensures in-house ancillary medical services, such as podiatrist, doctor visits, dental visits, psych visits, lab, X-ray, ambulance, etc. are scheduled and followed through.
  • Ensures family members are aware of resident's need for scheduled medical, dental, or eye appointments.
  • Provide medication administration to residents
  • Ensure accurate medication records and supply of medication at all times
  • Conduct medication cart audits
  • Assist care associates with personal care

Job Requirements

  • Must be able to multi-task and work in a fast paced environment.
  • Excellent communication and interpersonal skills and the ability to work as a team.
  • Previous experience in long term care or senior care preferred, but will train qualified applicants who possess the above mentioned skills.
  • Must be punctual and have a good attendance history.

Position Available

4 PM - 12AM (2nd Shift)

12AM - 8AM (3rd Shift)

(Weekdays/Weekends when scheduled)

Job Type: Full -Time

Hours per week: 30-40

Pay: $20.00 - $24.00 per hour

Weekly schedule:

  • Monday to Friday
  • Weekend availability

Ability to commute/relocate:

  • Albany, GA 31721: Reliably commute or planning to relocate before starting work (Preferred)

License/Certification:

  • LVN (Preferred)
  • BLS Certification (Preferred)
  • LPN (Preferred)
