Summary

The Triage/Scheduling LPN assists and is accountable for providing individualized Home & Supportive Care nursing care based on established standards of nursing practice and agency policies and procedures. This requires assessment, planning, implementation and evaluation through interventions based on clinical knowledge and skills. Patient care is delivered to patients between the ages of adolescent to geriatric. The Triage/Scheduling LPN is assigned to the Home & Supportive Care office to provide patient care telephone triage, patient referral coordination, and clinical scheduling support.

Essential Duties and Responsibilities

These duties and responsibilities described below represent the general tasks performed on a daily basis; other tasks may be assigned.

  • Provide telephonic patient care triage and communication:
    • Respond to phone calls in a prompt and courteous manner
    • Document all phone calls, clearly stating the nature of the call and the response(s) taken
    • Aid patients and families as needed in times of stress or crisis
    • Provide support and guidance to patients, families, and staff, counseling as appropriate
  • Dependent upon patient care staffing needs, may occasionally be assigned to participate in providing skilled care or palliative/hospice care for a caseload of patients as assigned by supervisor and under the direction and case management of an RN.
    • Clinical documentation is completed according to agency standards and Medicare guidelines. Documentation will reflect patient/caregiver response to plan of care, skilled treatment and intervention.
    • Performs nursing visits and nursing procedures in compliance with agency policy/procedure and regulatory standards:
    • Evidence of multidiscipline coordination of services is included in charting and participation in weekly case conferences.
  • Support clinical staff scheduling of patient visits and encounters:
    • Perform staff assignments of scheduled visits and patient encounters, managing staff schedules within the EMR system.
    • Participate in the assessment of staffing needs and call-in replacement personnel when necessary
    • Adjust staff schedules throughout the day, as need arises and perform reassignment of patient encounters as needed.
    • Accept and process referrals as appropriate for admission of patients to Home & Supportive Care services from the referral source.
  • MD verbal orders are to be written upon receiving orders. Documentation of time, date and who spoke with in the physician office necessary.
  • Contributes to assure that established departmental objectives are accomplished.
  • Prioritizes and organizes work in such a manner as to achieve maximum efficiency and high level of productivity.
  • Participates in coordination of patient care activities.
  • Teaches the patient/family about the patient's care needs disease process and health maintenance measures.
  • Maintains effective communication with co-workers and other referral sources.
  • Functions effectively in stressful and emergency situations.
  • Able to handle unexpected changes in workload effectively with the quality of patient care as the priority.
  • Acts as a liaison between patients, families and health care providers.
  • Maintains confidentiality and privacy of all protected health Information.
  • Recognizes and assumes patient advocacy role
  • Attend required hospital-wide orientations, meetings, and in-services
  • Demonstrate a commitment to flexible work scheduling when necessary to ensure patient care
  • Before discharging of a patient from nursing services, must discuss the patient’s status with the nurse case manager and document accordingly.
  • Reports to Nurse Manager

Education and Experience

Minimum Required Education: Graduation from an accredited LPN program

Preferred Education: Not applicable

Experience: Two-years medical/surgical experience preferred, home health experience preferred. New graduates are considered.

Certificates, Licenses, Registrations

Applicant must be licensed to work as a LPN in the Commonwealth of Virginia.

Must have and maintain current BLS Certification.
