General Summary: Provides safe, reliable and courteous transportation service to members; operates and maintains assigned vehicle(s); and maintains vehicle and other required records, including documentation.

JOB RESPONSIBILITIES:
  • Receives and plans route assignments 24 hours prior to providing transportation services.
  • Reads and interprets maps and driving directions to plan the most efficient route service for members’ transportation.
  • Assists passengers in and out of the vehicle; operates ramps, lifts and security devices as needed.
  • Presents safety briefing to passengers prior to each trip departure; ensures continuous use of safety belts.
  • Responds immediately to accident or medical emergencies by notifying emergency response providers and rendering First Aid until emergency personnel arrive.
  • Operates assigned vehicle in a safe and courteous manner.
  • Prevents smoking, eating or drinking in the vehicle.
  • Keeps the assigned vehicle(s) clean, inside and outside
  • Performs minor maintenance tasks on assigned vehicle(s) as required.
  • Fuels the assigned vehicle(s).
  • Maintains accurate up-to-date records on vehicle inspections, vehicle maintenance, fuel purchase, incident, accidents and other records as requested by supervisor.
  • Coordinates the schedule for major or periodic vehicle maintenance to minimize service interruptions.
  • Maintains trip logs, gathering the required supporting documentation required by funding sources, and enters documentation into the Electronic Health Record.
  • Completes all required training. Maintains an approved schedule and acceptable level of attendance.
  • Performs other duties as assigned or necessary as they relate to the nature of the position.
QUALIFICATIONS:
  • Education – High school diploma or GED required.
  • Experience – Prior experience as a driver preferred, including familiarity with vehicle maintenance needs and ability to perform minor vehicle repairs preferred.
  • Proof of COVID-19 vaccination required.
Regulatory

  • Minimum 21 years of age.
  • Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
  • CPR, First Aid & AED certification, if required (must possess upon hire and maintain throughout employment).
  • Valid Arizona Driver’s License, 39-month Motor Vehicle Report, proof of vehicle registration and liability insurance that meet company insurance requirements, if required.
  • Negative TB test result, if required (Employer provides).
