Position Details:
Devereux Advanced Behavioral Health Arizona's Outpatient program offers comprehensive outpatient counseling services for individuals, families, and groups in the office, school and/or home setting. Specialties include the treatment of Autism Spectrum Disorder, ADHD, Conduct Disorders, Mood Disorders, Physical Abuse, Sexual Abuse, and Anxiety Disorders.
Located in North Phoenix, the Transition Facilitator provides mentoring and independent living skills, as well as training on a one-to-one basis with individuals in the Helping Youth Participate and Excel (HYPE) program. This position provides family support and engages families in a healthy and positive manner while providing guidance and coaching to help them navigate the transition to adulthood.

Schedule: Tuesday-Saturday 8:00am - 5:00pm (flexible schedule)

Qualifications:
- High School Diploma/GED required
- Related experience preferred
- Bilinqual preferred but not required
OTHER:
- Must be 21 years of age or older
- Current AZ Fingerprint Clearance Card with no restrictions (OR eligibility to obtain with no restrictions)
- Pre-employment Physical and Job Demand Assessment (if applicable)
- Drug test within 24hrs of offer w/ negative results
- Must be able to complete and provide proof of current influenza vaccination
- Must attend the one (1) week paid New Hire Orientation; Mon-Fri (8:30am-5:00pm) follow by 3 weeks of on the job training
- Devereux requires all individuals interviewing for a position involving contact with children to complete The Diana Screen, a screening that assesses a candidate’s understanding of appropriate boundaries between adults and children
