Dimondale’s Aftercare Program provides adolescents transitioning out of Dimondale’s STRTP and into their next placement with ongoing access to mental health services. During this critical transition, we facilitate interpersonal skills and promote permanency and stability across relationships and living situations. The mission is to address the youth’s transitional experiences and heal past experiences of unexpected, unpredictable, and traumatic relational losses.

Summary

Our Aftercare Therapists are responsible for providing crisis intervention, intake assessment, and individual, family, and group therapy to adolescents with a history of mental illness and at risk of abuse, neglect, or both with severe emotional difficulties.

Duties include but are not limited to

• Maintain a remote hybrid schedule and perform in-home services with adolescents for treatment on an as need basis.

• Provide coordination of mental health treatment, which may include interfacing with community psychiatric hospitals, Department of Family and Children Services (DCFS), Department of Mental Health (DMH),, school officials, Regional Center, and school districts.

• Providing group treatment, parent support groups, and parenting education classes; also, emergency crisis intervention on an as needed basis.

• Demonstrates competency in managing crises with minimal guidance.

Requirements

• Master’s level and experience that includes working with individuals with a history of mental illness in social service. (MFT, MSW, LMFT, LCSW, Psy.D with state waiver and/or Licensed Psychologist required)

• Must be registered with Board of Behavioral Sciences (BBS).

• A working knowledge of conducting individual counseling sessions for those with a history of mental illness and their families.

• Strong working knowledge of crisis intervention and individual counseling, documentation, psychiatric intern supervision, and multicultural and socio-economic issues.

• Must be available for on-call duties.

• Valid California driver’s license and a driving record acceptable to the Agency’s insurance carrier.

Job Type: Full-time

Pay: $65,000.00 - $67,000.00 per year
