We have an opening for the Lead Case Manager with skills and a level of experience addressing housing instability in Multnomah County. Our programs are for individuals who are moving towards sustainable living.

Qualifications

  • A Bachelor’s Degree in a human/social services field; may also be an Associate’s degree in a relevant field with one year of field experience, or 3 years of human services work experience can be substituted for a degree in social work.
  • Basic computer skills with ability to maintain up to date and thorough records.
  • Demonstrable understanding of drug and alcohol issues within the Harm Reduction Model
  • Skillful communication with a working knowledge of Trauma Informed Care practices
  • Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in multi-cultural settings

Preferred Qualifications

  • Experience working within the supportive shelter system and/or resident setting.

Key Responsibilities

  • Provide compassionate and responsive supportive services to a caseload of about 15 Participants.
  • Work alongside each resident to develop individualized housing service plans to address their identified needs.
  • Coordinate and connect residents to services, including primary care, substance use treatment, mental health services, crisis services, and more.
  • Document case management interactions in the form of progress notes that reflects the resident’s goals
  • Provide crisis de-escalation and risk assessments for residents as needed
  • Collaborate with other staff and service providers to maintain the resident’s housing stability, safety and well-being
  • Create and facilitate activities to promote community building within the village community and in the broader community..
