Starting Annual Salary: $67,992 -$71,508

Can work remote or hybrid (home and office)

Duties:

This position would support contracted providers, with a special emphasis on those serving

Sandhillls’ high-risk members in an effort to reduce the separation rate (housing loss) per

requirement. Other assigned job duties would include but not be limited to:

  • Convene monthly collaborative meetings with ACTT and CST providers to provide education on the

aspects of tenancy needed to maintain TCL members in housing, such as community inclusion,

developing a meaningful day, meeting with members at the frequency/intensity needed;

  • Provide technical assistance to all TCL providers (ACTT, CST, TMS) regarding high-risk TCL

members, with particular emphasis on those individuals at risk of losing their housing, e.g., being

evicted or returning to an ACH in attempt to prevent the separation.

  • Collect TCL Provider Monitoring tools each month, enter the data from the tools, and analyze the

data to determine areas of need on an aggregate as well as provider-specific level:

Convene monthly collaborative meetings with ACTT, CST, and TMS providers to provide education

on the aspects of tenancy needed to maintain TCL members in housing, such as community

inclusion, developing a meaningful day, meeting with members at the frequency/intensity needed;

o Monitoring results will be shared with the TCL team and Network Operations to

determine areas where additional support is needed;

o Areas of need as shared across providers will be further reviewed/discussed during

collaborative meetings and technical assistance will be provided, as warranted, to

address the deficits.

  • Collaborate/ partner with the OT and RN as part of the complex care management team for the

purpose of working with high-risk members who are transitioning to the community to include

support to the member as they attain permanent supportive housing;

  • Review all person-centered plans (PCP) as needed to ensure that members are receiving services at

the recommended frequency/intensity and that interventions are recovery-based. In addition, the

LCC will complete monthly audits of TCL PCPs to ensure they are in agreement with the tenancy,

employment, and clinical needs of the members.

Minimum Requirements: Must be a Master’s level fully licensed as a LCSW, LCMHC, LPA or

licensed as and RN. Must have two years of clinical behavioral healthcare experience. Must have one

year of relevant experience working directly with individuals with SMI. Must have relevant and direct

experience providing diversion services under TCLI. Must reside in NC.
