Position Summary:
To ensure that GWH Team Members receive services from WFD, strategic partners, and other community resources consistent with the mission, identified
outcomes and the Team Member’s individualized plan that outlines a move toward retention and growth.
Under the direct supervision of the Mission Services Supports Manager, the Success Coach, is responsible for a variety of roles in order to provide individual
Team Members, and when warranted their families, comprehensive, “single point of contact” support as part of a seamless service delivery design. Services
to the team member include life and work coaching, personal and career counseling, work assessments, mediation, coordination and advocacy with
organizational and community resources, and providing case management with a goal of achieving self-sufficiency.
Essential Duties and Responsibilities:
 Accountable for the full Team Member cycle, including assessing, planning, implementing, monitoring and evaluating actions required to meet the team
member’s accommodation, retention and career growth needs.
Effectively develop positive empowering relationship with all Team Members through coaching, motivating, and educating.

 Actively promote GWH programs to create awareness and demonstrate the potential it has on career and life advancement.

 Be fluent in the evidence-based career navigation model and the specific career pathways within GWH.

 Be fluent in local community resources and programs and provides constant referrals to programs that foster continued growth of the Team Member.
Maintain working knowledge of educational and training opportunities located inside and externally in the community. Provide direct case management
services in accordance with policies, procedures, and standards associated with GWH WFD to further the success of the Team Member through the use
of Individual Development Plans.
Document Team Member contact and activity through case notes in electronic case management systems.

 Coordinate with Team Members and pre-schedule meetings.

 Refer Team Members to outside sources or additional training services or social aids.

 Be able to establish a rapport with Team Members.

 Intervene in crises.

 Maintain accurate records.

 Travel to multiple locations as needed.

 Other duties as assigned by Management.
Additional Responsibilities:

 Ability to multi-task, make quick decisions, adapt to change and work in a team oriented, fast paced environment.

 Adheres to all GWH Policies & Procedures, Loss Prevention & Safety Regulations and GWH Guiding Principles.

 Must be punctual and have dependable attendance and maintain an appropriate appearance.

 Must conduct work in a safe manner; maintain good housekeeping standards, have all aisles and pathways to fire exists clear and free
from clutter, keep work areas neat and clean.
Adheres to local, state and federal laws and regulations.
Minimum Qualifications:
 High school diploma or equivalent is required.

 One (1) year experience in case management is preferred.

 Extensive knowledge of community resources per the 4 sector areas, business, government, non-profit and faith based.

 Experience as a facilitator, instructor or career coach.

 Good computer skills (proficient in Microsoft Office).

 Excellent written and verbal communication skills.

 Ability to communicate and interact well with a variety of personality types.

 Ability to analyze opportunities and problems, identify and evaluate alternatives and develop effective approaches to address any issues.

 Ability to plan, implement and evaluate the achievement goals, objectives and work-plans.

 Must maintain a high level of confidentiality.

 Must have reliable transportation.
Critical Performance Factors:
 Serve GWH Team Members in assigned Area(s).

 Assist one (1) Team Member (minimum) in obtaining a high school diploma or GED.

 Ensure 85% of the Team Members reach a 30-day retention. Provide wrap around service referrals to 90% of Team Members
(cohort rollout determined by HR/DGR/WFD greatest need) to achieve a minimum of one (1) short-term goal identified in the needs assessment (IDP).
Complete and effective budget with the Team Member.

 Provide all Team Members on the roster with referrals for co-enrollment into GWH Programs.

 Execute WFD projects as assigned.
Physical Requirements/Work Environment:
 Ability to sit, stand, bend and reach.
 Ability to lift, push, pull, up to, 25 lbs.
