Preferred Transitions Home Health Care is looking to fill multiple positions for a Monday through Friday Start of Care Nurse.

RN with Home Care experience only

This job would entail:

Working 8 hour shifts, completing 2-3 admissions per shift.

32-40 hour position

Working Monday through Friday

NO WEEKENDS!

Rates: EXTREMELY competitive rates/salary

Responsibilities:

• Assessment of patient and conditions as per homecare guidelines
• Collaborate with physicians and nurses to devise individualized care plans for patients
• Perform Start of Care visits as need for patients

2.) Milford, Bellingham, Needham, Blackstone, Whitinsville, Attleboro, Franklin.
