Coastal Health & Wellness is seeking a Bilingual Social Worker!

The medical Social Worker will coordinate and provide social services for patients of all ages within the healthcare setting and in patient homes as needed. The services will be provided in accordance with the philosophy, mission, standards, policies, and procedures of Coastal Health & Wellness and as ordered by the physician.

Required:

  • Master’s degree in social work
  • At least one year of social work experience in a health care setting with home care experience preferred.
  • Valid driver’s license and own transportation to make home visits as needed.
  • Experience working with target populations of the Health District and the CHW Clinic or similar populations
  • Broad knowledge of Galveston County community resources for underserved
  • Knowledge of Case Management processes and resources
  • Knowledge of public assistance programs and eligibility requirements
  • Must be familiar with medical terminology
  • Bilingual in Spanish required
  • Ability for spur of the moment transportation without relying on organization vehicles.
  • Department of Motor Vehicle check may be required, if applicable.
  • Customer Service Oriented.
  • Excellent interpersonal, written and verbal communication skills.
  • Excellent Computer Skills MS Office.
  • Must be willing and able to work evening and weekend hours if necessary.
  • Must be in compliance with GCHD Immunizations policy.
  • Must be in compliance with ICS training requirements.
  • Must pass criminal background check and drug/alcohol screening.

An equivalent combination of education and work experience which appropriately demonstrates the knowledge, skills and abilities to perform the above described essential functions will be considered when hiring for this role.


Traditional Duties

  • Develops and maintains the social service program, drafting supporting policies, procedures, and guidelines for the program.
  • Develops and maintains a thorough knowledge of referral resources throughout the service area.
  • Makes home visits to assess patients’ status as needed. Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems.
  • Maintains accurate and timely records including clinical and progress notes according to department policy.
  • Works closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the assessment and accepted social work interventions.
  • Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
  • Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
  • Administer Social Determinant of Health survey, follow patients and document progress. Assist in creating workflow to operationalize and bill for services.
  • Participates in planning and implementing quality improvement activities.
  • Assists with contacting patients from the weekly hospital ER referral list to offer patients hospital follow up appointments at CHW
  • Assists with the monthly hospital referral report and the monthly/quarterly Case Management report
  • Assists with follow up calls to patients referred to the ER from CHW and documents outcome
  • Provides Prescription Assistance Program assistance as needed
  • Creates reports as requested
  • Answer patient inquires via phone and in person
  • Adheres to department/hospital policies
  • Performs other duties as assigned by supervisor
