An exciting opportunity has arisen for a motivated and enthusiastic Social Worker to join The City and Hackney Community Mental Health Team for Older People

The Team provides person and family-centred care for older people aged 65 years and over with complex and severe functional mental illness (including schizophrenia, Bipolar affective disorder, Emotional Unstable Personality disorder, depression, anxiety) and organic illness - Dementia living in City and Hackney area of London.

This role requires experience of working within a mental health setting combined with experience of working as part of a multi-disciplinary team. Experience of making full psychosocial assessments and writing reports is essential.

For this role, you must be a qualified Social Worker- CQSW/DIPSW and be registered with Social Work England. Evidence of continual professional development is key to this role.

Social Workers within community mental health care for Older People are embedded within the Mental Health Teams which requires them to deliver the statutory duties under the Care Act 2014.

Social Workers will be working with members of the multidisciplinary team including Mental Health Nurses, Doctors, Psychologists, Support Workers and Occupational Therapists.
