Position Summary:


Facilitates the educational development and professional growth of assigned Coordination of Care personnel. Responsibilities include development and implementation of orientation, in-service and continuing education for the staff which focuses upon the scope of service, emphasizing the age, psychosocial and physical needs of the patient population. Responsible for coordinating and maintaining Coordination of Care Policies and Procedures.

Knowledge, Skills, and Abilities:

• Critical thinking and clinical competence demonstrated at above average level. • Excellent interpersonal communication and negotiation skills. Able to communicate effectively and work with people of all social, economic, and cultural background. • Ability to discuss a patient’s clinical, socio-economic, and financial issues with physicians and patient and/or patient’s representative. • Ability to understand directions and communicate and respond to inquiries in an accurate manner. • Knowledge of insurance guidelines, federal regulations and community resources required. • Self-motivated, proven written, telephonic and electronic communication skills, assertive and persuasive in interactions with customers, peers, management and core staff served. • Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components. • Proficiency with various computer programs, to include Microsoft Office and EPIC. • Ability to input and retrieve information from computer, operate fax, phone, and copy machine. • Skills necessary to build credible and effective relationships with physician leadership, the internal and external customer. • Flexible, open-minded, and adaptable to change. Models positive change. Ability to demonstrate respect and team building. • Able to prioritize and process multiple tasks, responsibilities, and work projects, demonstrating strong organizational and time management skills. • Demonstrated ability to analyze related information, plan effective actions and follow through reliably to meet expectations. • Ability to work and promote interdependence demonstrated in actions resulting in sound judgment with interactions with physicians, peers, patients and /or designee, payors, support service personnel. • Ability to work collaboratively with department staff, physicians, and healthcare professionals at all levels to achieve established goals, improve quality of outcomes, maintain, or exceed Joint Commission standards and state mandates as they apply to the department operations.

Required Education, Licenses, and Certifications:
  • Master's Degree Social Work
    Must have a minimum three to five years’ experience in Case Management.

Preferred Education, Licenses, and Certifications:
  • LICENSED CLINICAL SOCIAL WORKER
  • CERTIFIED SOCIAL WORKER

Other Information:

Some light carrying, and lifting may be required. Must be able to move about hospital and between workstations. Occasional walking may be required to access all areas of the Medical Center. Ability to effectively communicate orally to patients, family members, personnel, and physicians. Must have intact sense of sight and hearing, finger dexterity, critical thinking, and ability to concentrate. Must be able to respond quickly to changes in assignments. Position involves sitting for extended periods of time performing data entry into a computer. Requires minimal travel between various campuses.
