Site Leader Position:

PRAC of SNJ Mobile Vaccine Team CHW Site Leader for Burlington County.

Position information:

Full-time, non-exempt, 35-40 hours a week

Pay range: $22-$25/hour, depending on experience.

The Puerto Rican Action Committee of Southern New Jersey is seeking a CHW Supervisor to assist in the planning and execution of COVID-19 mobile vaccine clinics in the community. The

Community Health Worker Supervisor will supervise and lead a small team of CHWs to provide public health outreach services to local communities and perform non-clinical operational roles for mobile vaccination clinics. This position will require community visits and in-person work at vaccination clinics. Reliable transportation to on-site clinics and outreach events is required.

Must be able and willing to move clinic supplies and outreach gear to support operations of community health events. The progression of the epidemic response, the responsibilities of direct support of COVID-19 mobile vaccination clinics will include other aspects of community engagement, outreach, and education as needed.

• Supervise and lead a small team of CHWs during in-person outreach events and vaccination clinics.
• Perform non-clinical roles in support of COVID-19 mobile vaccine events, including but not limited to: o Set up and break down of mobile vaccination sites, including equipment, supplies and signage. o Transportation of Clients o Patient registration and other administrative or clerical needs o Providing basic health education around COVID-19 and COVID-19 vaccines o Directing flow of people through clinical spaces o Promoting and enforcing COVID-19 safety protocols o Documenting and tracking supplies and materials o Timing observation periods for patients o Sanitizing surfaces and maintaining a clean and safe vaccination space
• Coordinate pick up and distribution of materials and supplies.
• Lead team members to conduct canvassing, outreach, and educational events to vulnerable communities across local communities to promote COVID-19 vaccination and mitigation strategies.
• Support team members in registering patients for COVID-19 vaccination using the preferred computer platform.
• Track clinic data and patient rosters while ensuring appropriate data confidentiality and protections.
• Provide appointment reminders and follow-up with clients over the phone.
• Participate in program trainings and workgroups as needed.
• Assist with other community outreach tasks and community education events as required.

QUALIFICATIONS:

• A Master and/or bachelor’s degree from an accredited college or university is preferred. Having three years of experience providing community health promotion, outreach, or services or working in a home or community setting is preferred.
• Knowledge of local health disparities and resources to address those disparities.
• Reliable transportation, valid driver's license and insurance required. (Mileage reimbursement provided for on the job use of vehicle)
• Spanish Speaking preferred not required.
• Other applicants with equivalent public health work experience and training may be considered and complete CHW training while on the job.
• Strong computer literacy skills, specifically with MS Word, Excel, Outlook, and SharePoint (MS Office 365).
