The Service Coordinator position can be performed remotely and will require travel within Dauphin County.

The Service Coordinator position can be performed remotely and will require travel within Dauphin County.
- Development of the person centered service plan that address Participant identified areas of need, goals, intervention, and progress of goals
- Coordinate services for Participants in relation to their identified needs
- Monitor authorized services through phone calls, face-to-face visits, person centered planning team meetings, and contact with providers
- Service Coordinators must, have a Bachelor’s degree in social work, psychology, or other related fields with practicum experience, or in lieu of a Bachelor’s degree have at least three (3) or more years’ experience in a social service or healthcare related setting. Service Coordinators hired prior to the CHC zone Implementation Date must have the qualifications and standards proposed by the CHC-MCOs and approved by the Department.
