Job description
Glen Brook Rehabilitation & Nursing Center in beautiful Berwick, PA is currently hiring for a Social Services Coordinator. This is a fantastic facility to join a growing team at Glen Brook in Berwick, PA.
The hourly rate range is between $23-$30 per hour depending upon experience.
Job Duties and Responsibilities:
- Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.
- Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.
- Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placement
