This is a hybrid position (3 days/week in the office, 2 days/week at home). Our office is located at 12921 S. Vista Station Blvd. It also includes occasional field work in the local community, meeting with members in hospitals and other facilities. The schedule is Monday - Friday 8 AM - 5 PM.


Primary Responsibilities:

  • Reach out to members who have been hospitalized within 72 hours of discharge to ensure an aftercare plan is in place and scheduled
  • Help link to services if aftercare has not been arranged by a hospital
  • Follow up with provider and member after scheduled appointment to ensure aftercare is a good fit
  • Assess social determinants of health barriers and work to resolve with the member so that they stay engaged in treatment
  • Help educate hospitals about Optum's provider network and treatment resources
  • Meet with members in the hospital if the hospital is having a hard time engaging the member for aftercare
  • Work with ACT teams to enroll appropriate members with them
  • Reach out to Homeless Resource centers to locate members who have not engaged and use motivational interviewing to try and engage them in treatment
  • Assist with Certified Peer Support trainings as needed
  • Take calls from the queue when a member would benefit from speaking with a Peer

Required Qualifications:

  • High School degree or equivalent experience
  • Certified Peer Support Specialist OR the willingness to become one
  • 5+ years of experience working in the mental health field
  • Experience working with persons experiencing Serious Mental Illness
  • Knowledge of Salt Lake County resources
  • Demonstrated understanding of Medicaid
  • Familiarity with Motivational Interviewing
  • Willing to work a hybrid arrangement (3 days/week in the office, 2 days/week at home)
  • Reliable transportation and the ability to travel as needed within the designated service delivery area
  • Access to install secure, high-speed internet and a dedicated workspace at home
  • Reside in or near Draper, UT
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.
