Amazing Care Home Health Services is currently hiring RN Case Manager for Greeley, CO Home Healthcare Visits.
Coverage Areas:
Greeley, Evans and Surrounding areas.

Please note: Pay rate is PER VISIT not hourly.
Job Requirements:
- RN License Required.
- Home health experience required
- Ability to work independently.
- Excellent documentation skills with a solid grasp of computer usage.
- Provide effective communication to patient/family, team members, and other health care professionals.
