Home Health Agency seeking experienced RN to cover Cincinnati area in Ohio.

Reports to: Clinical Manager and Administrator

Supervises: Staff Coordinator and all direct patient care employees Education/Qualifications

  • Registered Nurse currently licensed in the State of Ohio. Bachelor of Science degree in Nursing preferred.
  • Minimum of three (1) years experience as a licensed, registered nurse in Home Health Care.
  • Experience in a supervisory or quality assurance position in acute and/or home health is better.

General Description: Provide quality patient care Through development, implementation & evaluation of individual patient care plans. Follows established professional standards of care, Agency policy and procedures.

DUTIES/RESPONSIBILITIES

  • Develops a Plan of Care that corresponds with the physicians orders through assessment of patient's needs, condition, environment, consultation with other Clinical team members, at time of initial visit and updates plan of care regularly as necessary and documents care plan following Agency procedures.
  • Responsible for the delivery of competent, quality patient care in the home.
  • Maintain an ongoing responsibility for assigned caseload.
  • Responsible for the implementation and evaluation of individual patient plan of care.
  • Regular communication with Case Managers, Staff Coordinator and Director of Nursing.
  • Provides teaching for both patient and family.
  • Provides skilled nursing care that conforms with Agency patient care and general medical policies.
  • Schedules work load for maximum efficiency.
  • Rotates weekend, night, and holiday call with staff to facilitate adequate coverage for all patients.
  • Participates in Agency evaluation program as instructed.
  • Regularly conducts Case Care Conferences.
  • Abides by and promotes Agency administrative policies.
  • Maintains a continuing knowledge of competencies related to the Nursing profession by participating in formal education programs, conference, workshops and professional organizations.
  • Participates in staff in service programs, at lest 12 hours annually.

Job Types: Full-time, Part-time

Pay: $60,000.00 - $72,000.00 per year
