What You Should Know About the RN (Register Nurse) Admission Care Liaison Role:
  • The RN Admission Care Liaison is a DRIVING position supporting Dayton and the surrounding areas
  • Schedule includes:
    • Four 10 hour shifts per week with every 3rd weekend and one holiday a year.
    • Hours will include 10am-8:30pm
  • We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed

The RN (Register Nurse) Admission Care Liaison Essential Duties Are:

Ohio’s Hospice patients will receive quality nursing care and coordination of hospice services, when the RN Admissions Care Liaison admits the patient into the hospice program efficiently and provides the patient and family with the appropriate resources needed at the end of life. The RN Admissions Care Liaison will maintain a conversion rate of 75% and skillful use of the electronic medical record by accurate documentation at the point of care.

Qualifications:

  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years acute care nursing experience preferred
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
  • CPR Certified
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
