Crossroads LTSR provides intensive and transitional care to assist residents with empowering opportunities to apply their acquired skills in learning to meet their own needs by seeking out supports and follow through with physical and mental wellness treatments. Residents are encouraged and supported in developing the natural resources and wellness plans to be successful after discharge.

The services we offer include:

  • Individual Therapy and Group Therapy
  • Creative expressive arts and music therapies
  • Weekly psychiatric evaluation
  • Medication management and medication education
  • Access to medical services and physical fitness activities.
  • Assistance and life skill development for activities of daily living
  • Provide psycho-educational supports with access to technology and social media.
  • Enhance social skill development and conflict resolution.
  • Facilitate recreational and therapeutic activities with access to an onsite physical fitness room.
  • Intensive case management services to assist in obtaining needed resources, make referrals and appointments while establishing continuity of care, securing housing and discharge planning.

The Residential Mental Health Therapist position provides a full range of counseling and case management services to adults living with mental illness.

Duties/Responsibilities: The following list indicates the general responsibilities of work performed by this position. It does not list a comprehensive inventory of all tasks required.
Provides individual and group counseling services for LTSR residents.
Continually monitors members’ personal safety and provides appropriate crisis intervention utilizing certified skills and techniques.
Conducts assessments and monitors members’ progress, both formally and informally for the purpose of providing quality treatment and tracking outcomes.
Participates as a vital member of the member’s Treatment Team, to include planning, development, documentation, and review of the Treatment Plan
Conducts discharge assessment and discharge plan prior to discharge date.
Plans, facilitates, and supervises psycho-educational groups, therapeutic activity groups, recreational and therapeutic activities.
Implements and monitors written behavior management and crisis intervention plans.

Knowledge, Skills & Abilities:

Education: Minimum: A master’s degree in human service field.
Experience: 1 to 2 years mental health experience in the mental health or related field preferred with adult individuals with psychiatric or developmental disabilities.
Certification: Ability to obtain and maintain CPR, First Aid, Infection Control, Fire Safety and Crisis Prevention and Intervention and other certifications required for employment.
Compliance: Ability to comply with Client’s Right, CRCSI’s policy and procedures. Ability to read, writes, and speaks the English language comply with Act 33, Act 34 and FBI clearance procedures. Be able to comply with pre-employment and biennial physicals, and with tuberculosis and hepatitis screenings.
Other Requirements: Basic computer skills. Have a valid driver’s license with the ability to legally transport members.
