Position Summary:

To provide quality nutritional care for all patients on specified units. Instruct patients, significant others and special groups on therapeutic diets and basic nutrition as prescribed by physician and surgeons. Works with other members of the patient care team to deliver care to patients in age groups including newborn, infants, children, adolescents, adults, and geriatrics. Act as a resource person in the area of clinical nutrition for medical staff other hospital personnel and the community.

Education, License & Cert:

B.S. Degree in Food and Nutrition, Dietetics, Food Service Management or related sciences with completion from an Academy of Nutrition and Dietetics approved Internship or Coordinated Undergraduate Program. Registration with the Commission on Dietetic Registration, the credentialing agency for the Academy Of Nutrition and Dietetics preferred; registry eligible will be considered but candidate must obtain CDR RD within 12 months of hire

Experience:

Prefer 2 years clinical experience. Previous experience working with EPIC(EMR) and/or Cbord (Nutrition software system) also preferred

Essential Functions:

1. Nutrition screening and assessment of individual patients a. Supervise nutrition screening of individual patients. b. Supervise nutrition assessment of individual patients with common medical problems, e.g. hypertension, obesity, diabetes, cardiac and diverticular disease. c. Assess nutritional status of individual patients/clients with complex medical conditions, i.e. more complicated health conditions in select populations e.g., renal disease, multi‐system organ failure, trauma. 2. Nutrition care of individual patients a. Manage the normal nutrition needs of individuals across the lifespan, i.e., infants through geriatrics and a diversity of people, cultures, and religions to include monitoring of patient’s food and/or nutrient intake. b. Reviews existing data in patient medical record including admission and correct diet order; history and physical, laboratory values that indicate nutritional status, and nursing assessment. c. Recommends the collection of additional pertinent data, as needed. d. Interviews patients and/or significant others for diet history, dietary preference and nutritional concerns. e. Design, initiate and implement nutrition care plans as indicated by the patient’s health status that are thorough, concise and done in an organized manner. f. Modifying diet orders through delegated authority from the medical staff is the responsibility of the RD as part of a documented nutrition assessment. g. Determine appropriateness of prescribed diet therapy. Recommend changes as necessary or modify order if appropriate as delegated by patient’s physician. h. Review and evaluate enteral & parenteral nutrition regimens, to determine if nutritional requirements are met and make recommendations/adjustments that involve primarily macronutrients. i. Develop and implement transitional feeding plans, i.e. conversion from one form of nutrition support to another, e.g. total parenteral nutrition to tube feeding to oral diet. j. Consults with patient, physician and other hospital personnel to reevaluate dietary regimen to reflect changes in requirements. k. Conduct nutrition care component of interdisciplinary team conferences to discuss patient care and discharge planning. l. Initiates and individualizes the nutrition intervention. 3. Nutrition education of individual patients a. Develops and updates patient nutrition education materials in their area of expertise. b. Provide or supervise counseling, education, and/or other interventions in health promotion/disease prevention for patients needing medical nutrition therapy for common conditions, e.g. hypertension, obesity, diabetes, cardiac and diverticular disease. c. Refer patients/clients to appropriate community services for general health and nutrition needs and to other primary care providers as appropriate.
