We are hiring a Recovery Support Specialist-Transitions, to provide patients with the natural supports proven instrumental in maintaining a strong and successful recovery and will demonstrate a strong personal recovery and be a positive example that recovery is a desirable and attainable goal. Works in conjunction with multiple disciplines to meet patient’s treatment and recovery needs.
Responsibilities:
  • Acts as an active facilitator (or co-facilitator) in groups and individual recovery. Serves as a patient advocate and support resource throughout the treatment process.
  • Works to lessen the instance of seclusion and restraint by building therapeutic alliance and exercising trauma informed care.
  • Willing to travel as needed to present Connections Health Solutions in a professional manner within the community.
  • Collaborates with community stakeholders and demonstrates knowledge and accessibility of community resources to help meet patient’s unique recovery needs.
  • Has an aptitude for engaging community stakeholders, influencing multiple decision makers and finding creating ways to bring value, and understanding to complex interactions.
  • Must be able to relate well to people both inside and outside the organization; build appropriate rapport; uses diplomacy and tact; can diffuse high-tension situations.
  • Model appropriate interpersonal relationships, emotional regulation and boundaries.
  • Relate professionally, ethically, and compassionately to clients, peers, and staff at all times.
  • Ability to implement program guidelines, regulations, policies, and procedures, along with knowledge of multiple paths of recovery and promotes them equally.
  • Works cohesively with multi-discipline team through all aspects of patient’s treatment and recovery.
  • Documents all encounters with individuals in the electronic health record.
  • Encourages and supports from a place of experience and understanding and willing to use appropriate self-disclosure.
  • Provides community based supportive services, to include driving to conduct home visits and transporting individuals to appointments as needed.
  • Performs all other duties as assigned.

Minimum Qualifications:
  • The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment.
  • Ability to work independently, self-motivated and able to implement program guidelines, regulations, policies, and procedures.
  • 1-year of workplace experience within: Behavioral Health, Recovery Treatment or GMH Recovery.
  • 12 consecutive months of personal Recovery experience.
  • Knowledge of evidence-based/person-centered skillsets, such as; Motivational Interviewing, Trauma Informed Care, Verbal De-escalation, Harm Reduction, and Person-centered language.
  • High School Diploma or equivalent.
  • Arizona Peer Support Specialist certification.
  • Valid driver's license.
  • Successfully pass a motor vehicle record check.
  • Must have own vehicle that is clean, neat and in good working condition.
  • Willing to obtain full coverage car insurance with high-level liability.
Preferred Qualifications:
  • Bilingual in Spanish.
  • Experience using EMR.
  • Completion of specialized peer certifications: WHAM (Whole Health Action Management), ASIST (Applied Suicide Intervention Skills Training), SMART (Self-Management and Recovery Training), etc.
  • Able to create SMART goals.
