Are you looking for a role in an inclusive, supportive and friendly organisation? Are you looking for a job that uses creativity and makes a real difference to people who are struggling?

Sefton CVS, South Sefton and Southport and Formby CCG’s are currently delivering a project to support high intensity use of emergency NHS services, known as ‘frequent attenders’.

We are excited to announce that this service is expanding and we are now seeking a number of incredible and forward thinking people to join our team, who can connect with people and provide 1:1 coaching to people who are going through difficult times. The post holders must have two dominant skills

  • high emotional intelligence; to be able to connect with people quickly
  • be an excellent negotiator; with services and professionals as well as service users

The successful candidates will work across Sefton with a rolling cohort of patients and a varied caseload, supporting people who feel they have nowhere else to turn but A&E or their GP surgery, more than expected. The aim of the programme is to provide a de-medicalised, human approach that doesn’t punish people for being in crisis. Evidence shows that if we concentrate on people and what they need, the results follow and healthcare contacts naturally fall as people get the time, understanding and solutions they need.

You will work with a team of other colleagues with a range of skillsets to support people to navigate their way through the complex health and social care environment, removing the barriers that may impede self-management and helping to embed coping strategies.

The role will require you to build strong relationships with the Sefton community, healthcare professionals, working closely with GPs, link workers and other multi-disciplinary teams to facilitate joint working to ensure that creative, sustainable and person-centred support is given.

If you are interested in applying for this post, please complete the application form, including how you meet the criteria in the person specification.

The post is initially funded for 12 months and is subject to an enhanced DBS check and satisfactory references. Your health and happiness are important to us, so we offer a comprehensive staff wellbeing package.

This is a full-time role working 35 hours a week, in line with service requirements and agile working arrangements.

Applicants will be shortlisted upon us receiving completed applications forms and interview dates confirmed after this.
