The QIDP (Qualified Intellectual Disabilities Professional) is responsible to the ICF/ID Manager for the coordination of individual service plans for the assigned caseload of person served in the ICF/ID program. The QIDP supervises designated Leads and Recreation Coordinator. This position is full time, Monday - Friday 8:00a - 5:00p, with on call responsibilities.

Qualifications:
1. Possess a bachelor's degree in human services related field or a license as a Registered Nurse.
2. Possess minimum of one year experience specific to working with persons with Intellectual Disabilities and/or other developmental disabilities.
3. Valid Class C driver’s license required and be an insurable driver with our insurance company.
4. Ability to pass a health assessment at time of hire and ability to maintain up to date health assessment as requested per regulations.
5. Live within 30-minute driving distance of company preferred.
6. Clearance from a criminal background check and abuse reporting check at time of hire.
7. Comply with ICF/ID CMS health requirements.
8. Reliable Transportation.
9. Provide proof of state minimum liability insurance coverage.

Duties and Responsibilities:
1. Facilitate initial and annual Individual Service Plan (ISP) staffing’s in coordination with members of resident’s Interdisciplinary Team
2. Utilize a team process to develop resident’s programs in collaboration with ICF/ID leadership and IDT members.
3. Coordinate services and programming with contracted consultants, including therapists and licensed psychologist.
4. Approve and sign all resident’s formal written programs and procedures.
5. Monitor program effectiveness through 10 hours of direct observation weekly and interaction with program staff, monitoring and collaboration with the home leadership to assess resident’s progress and effectiveness, and to ensure accurate implementation of resident programs, along with a review of records.
6. Ensure training is being done consistently and also make timely modifications to facilitate achieving desired skills or goals by identifying inconsistencies in training procedures and facilitate resolution of inconsistencies.
7. Maintain compliance with the Departments of Inspections and Appeals (DIA) regulations and participate the annual survey and plan of correction process.
8. Coordinate services and programming with the resident’s schools and other provider agencies to facilitate an interdisciplinary program for each resident served.
9. Coordinate with the Social Services department regarding out-of-facility consultations, evaluations and assessments for residents, as recommended by the IDT.
10. Serve as ICF/ID On-Call staff on rotating basis as scheduled.

Physical Requirements:
1. Requires physical mobility to walk, stoop, stand, sit, and climb steps.
2. Requires the physical strength to assist individuals in transferring as per Hope Haven policy.
3. Requires physical strength to restrain an individual as outlined in the SCM procedures.
