PT Nurse Practitioner

Job Summary

• Licensed independent practitioner who is responsible for managing health problems and coordinating health care in accordance with State and Federal rules and regulations and the nursing standards of care
• Provides specialty care and services in a range of clinical settings including but not limited to hospitals, dialysis centers, clinic offices.
• Assess, monitors, and documents patient’s needs and care provided.
• Collaborates with physicians and other health care providers to discuss patient’s plan of care.

General Accountabilities

• Functions independently to perform age-appropriate history and physicals for complex, acute, critical, and chronically ill patients with various kidney diseases.
• Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
• Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
• Implements and monitors interventions to support the patient in regaining or maintaining physiologic stability.
• Collaborates with multidisciplinary team members by making appropriate referrals.
• Appropriately reports patient’s status and conditions to the physician.
• Documents all assessments, treatments, therapies, diagnosis, and evaluations in an appropriate timeframe.
• Delivers compassionate support, attention, and assistance to patients and families.
• Ensures compliance with all health care regulations, including HIPAA and OSHA.
• Performs other tasks as assigned by the Physician and/or Clinical Manager.

Job Qualifications

Education:

• Graduate of an accredited Nurse Practitioner Program
• Current and active Advanced Practice Registered Nurse (APRN) Licensure
• Current and active Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification.

Experience:

• At least one year of experience as a practicing Nurse Practitioner preferred
