Community Health Connection, Inc., Tulsa, OK is a Federally Qualified Health Center (FQHC) committed to meeting the lifelong health care needs of the northeast Oklahoma community. At Community Health Connection we have an immediate need for a full time Centering Program Coordinator. This position will report directly to our Medical Director. We are seeking an individual with prior healthcare experience, program management, communication and outreach experience. Must be bilingual (English/Spanish). Role and Responsibilities Summary:

The Coordinator is responsible for administering and managing our Centering Parenting and Centering Pregnancy programs. This includes planning and organizing the group sessions, participating in community outreach events, and helping to recruit patients into the program through direct interactions, social media and other forms of outreach. Experience in the public health community programs, teaching early childhood education or nursing is preferred.

1. Coordinates all aspects of the Centering Parenting & Pregnancy Programs.

a. Facilitates Steering Committee and Core Team meetings.

b. Develops and implements procedures as directed by the Steering Committee.

c. Under the direction of the steering committee, identifies opportunities to promote Centering to patients, clinic staff and to the community at large.

d. Oversees the planning and set-up for groups sessions.

e. Actively participates in evaluation of program evaluation.

f. Assists with data collection and report preparation to meet requirements.

g. Collaborates with Medical Director to ensure program requirements are met.

2. Participates as a member of the prenatal team for non-Centering patients.

a. Performs nursing functions as part of the care team, provides lactation consulting and nutrition education and travels to delivery hospitals to set up follow-up appointments at CHC for newborns.

b. Regularly attends site and assigned meetings; including education sessions.

c. Identifies training needs of self or support staff to Medical Director.

d. Supports the competency and clinical development of other team members.

Job Type: Full-time

Pay: $43,000.00 - $51,000.00 per year

Schedule:

• 10 hour shift

COVID-19 considerations:
All patients and employees are required to wear a mask, temperature screenings, common surfaces are sanitized regularly, etc.
All employees are required to be vaccinated against COVID or undergo weekly testing.
