Centerstone Health Services, Inc. (CHS) is a non-profit 501(c)(3) Federally Qualified Health Center Look Alike that was designated by the U.S. Department of Health and Human Services, Health Resources and Services Administration in 2020. Centerstone Health Services serves the citizens of three counties (Bartholomew, Monroe, and Fayette) in south central and east central Indiana. Integration of services is at the heart of who we are. CHS provides primary medical, behavioral health, substance use disorder, and pharmacy services in three high-quality health care facilities (Bloomington, Columbus, and Connersville Indiana) to care for the whole person utilizing an integrated team of physicians, advance practice practitioners, social workers, psychiatrists, and psychologist. Our Mission is to embrace health and well-being for all people by providing comprehensive, accessible, quality health care regardless of income to promote wellness and restore health.

The Physician/Provider systematically assesses patient’s health status, arrives at appropriate diagnosis(es), and implements care plans for effective clinical outcomes.

Essential Duties and Responsibilities – Quality Improvement:

  • Considers thorough range of treatment/preventive options and arrives at a recommended course of action that is age appropriate, culturally appropriate, realistic, and attainable.
  • Appropriately manages patient care and health status following initiation of treatment/preventive plan, and monitors and adjusts as warranted.
  • Communicates effectively with patients and family so that they understand their diagnosis, treatment plans and need for follow-up care.
  • Educates and encourages patients to become an active participant in their own health care related behavior and provides patient education materials of use in encouraging wellness.
  • Seeks consultation and advice by providing adequate information to consulting physicians and communicates request effectively or responds to such requests from the referral source in a courteous, professional, and timely manner.
  • Documents and completes patient charts to facilitate ongoing quality of care in a timely manner.
  • Performs appropriate medical procedures within scope of practice as defined by the Centerstone Health Services Credentialing and Privileging processes.
  • Delegates directly and indirectly patient care activities to appropriate personnel.
  • Responds appropriately to emergent situations.
  • Maintains and improves skill level through participation in continuing medical education.
  • Actively participates in clinical quality improvement initiatives.
  • Takes actions to assure continuity of care for patients.

Contributes to effective & efficient practice management policies, practices support for patient’s access to medical care:

  • Participates in access planning for the clinic site, ensuring appropriate amount of same day patient access.
  • Meets number of clinic patient contact hours as outlined in the contract agreement.
  • Collaborates in establishing production numbers to be budgeted and achieves targeted goal.
  • Fulfills commitments for call assignments with clear communication and follow through on work initiated.
  • Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of the Centerstone Health Services, Inc. billing policies and procedures.
  • Adheres to all relevant policies, procedures, and practice guidelines.
  • Assists with the preparation for quality and third-party payor requirements.
  • Responds to telephone calls in a timely fashion.

Manages patient care through appropriate use of available resources:

  • Appropriately prescribes and orders ancillary services.
  • Utilizes referral network appropriately.
  • Promotes and appropriate length of stay for patients.
  • Manages chronic health/disease conditions.

Provides leadership for clinic and community relations:

  • Serves as a resource to colleagues and support staff, enhancing quality of care.
  • Participates in the training and evaluation of clinic support staff.
  • Regularly attends and actively participates in clinic staff meetings and provider group meetings.
  • Participates in appropriate committee structure as appointed.
  • Participates in community education and other professional activities to promote Centerstone Health Services, Inc., and the Health Center.

Education and/or Experience: Medical Doctor degree (M.D./D.O.) from a four-year medical school and three-years of residency. Required licenses and certifications include: Unlimited license to practice medicine in Indiana. Board Certification preferred.

Clinical Supervisory Responsibilities: Maintains collaborative practice or Delegation agreements with Advance Practice Practitioners (Nurse Practitioners/Physician Assistants).

Job Type: Full-time

Pay: $185,536.00 per year
