As a Hospice Physical Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a Physical Therapist (PT) include:

• Identifies safety risks and uses professional judgment in progressing patients through plan of treatment.
• Evaluates and recommends/performs Physical Therapy interventions for symptom control/palliation.
• Provides office consultations to the nurse of hospice IDG.
• Meets agency standards for timeliness and completeness of documentation and responds of referrals in a timely manner.
• Provides physical therapy, for hospice patients when requested by the physician and hospice IDG.
• Instructs patients/caregivers in the home exercise program for continued palliation between visits and in the care and use of equipment designed to accommodate for physical disabilities.

Qualifications for a Physical Therapist (PT) include:

• A current Missouri Physical Therapist - PT license.
• A minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
