Qualifications:

• Graduated from an accredited Physical Therapy program or university
• Valid professional license and registration for state of NY
• COVID vaccination mandatory per site requirements

Responsibilities:

• Complete evaluations, daily notes, weekly progress notes, recertifications, and discharge documentation in a timely manner and in accordance to CMS guidelines
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the physician
• Supervise assistants as requested by DOR
• Collaborates with all disciplines to provide best patient centered treatment and care
• Adhere to scheduled hours and demonstrate professionalism at all times
• Familiarity with EMR documentation (we use Rehab Optima)

Job Types: Part-time, PRN, Per diem

Pay: $55.00 - $60.00 per hour
