Registered Physical Therapist (PT) -PRN

Per the Massachusetts Department of Public Health, all employees of Saint Francis Rehabilitation and Nursing Center must adhere to the COVID-19 vaccination requirements.

SUMMARY

Provides skilled physical therapy services consistent with professional standards and policies. Participates in the development of programs necessary to achieve patient care objectives.

Education and Experience of the Physical Therapist (PT):

  • Minimum of a degree from an accredited Physical Therapy program for RPT.
  • Prefer a minimum of one year of experience in a therapist capacity.
  • Preferred experience in skilled nursing facility setting or rehab hospital.

QUALIFICATIONS of the Physical Therapist (PT):

Licensed as a Physical Therapist in the state(s) of practice.

Job Responsibilities of the Physical Therapist (PT) include:

  • Provides physical therapy in accordance with physician's orders and accepted professional standards of practice.
  • Performs patient centered evaluations to determine functional level and establish a therapeutic plan of care.
  • Develops an overall therapy plan of care to include treatment and medical diagnosis warranting treatment , the planned duration and frequency of therapy to be provided, a minimum of long-term goals and discharge plans; per standards of practice.
  • Utilizes standardized assessments upon the development of the plan of care to validate the need for skilled therapy services and demonstrate repetition of this standardized assessment to reflect the ongoing need for skilled therapy and the patient 's progress.
  • Documents observations, patient 's response to treatment, treatment programs provided, degree of progress and notable changes in patient's condition.
  • Completes all billing required on the date of the provided treatment, and in accordance with all state and federal regulations, and company policy.
  • Communicates and coordinates patient's care with other members of the patient' s health care team and the patient.
  • Supervises physical therapy assistants in accordance with regulations.
  • Provides patient, family and caregiver teaching related to the patient's physical therapy program, safety, and functional status.
  • Assist the patient and/or family to procure needed equipment or supplies for optimal functioning and safety both in and out of the home environment.
  • Hold the skills of and demonstrate strong time management and organization skills, as well as seamless communication.
  • Maintain a positive attitude within compliance to the company' s code of conduct.
