Job description

This position is as contract physical therapy and requires flexibility to travel to multiple locations. High energy, empathy and professionalism are essential to this role.

Area: Lakewood, Artesia, Cerritos, Downey, Long Beach, Norwalk, San Pedro, Palo Verdes, Whittier, Torrance, Redondo Beach, Bellflower, Carson, Signal Hill, Lomita, Maywood, paramount, Pico Rivera, Wilmington, Compton, Bell etc.

If you can cover few areas or all, take only one client a week or multiple, let us know. you make your own schedule and work at your own pace.

Responsibilities:

• Provide physical therapy services to patients according to referral from physician, assess and evaluate therapeutic and functional status, evaluate home environment, assess muscles strength, mobility, gait, ROM potential for rehab services, directing and providing physical therapy treatment.
• Supervises physical therapy assistants according to organization policy and state regulations
• Manage cases to ensure continuity of care.
• Document patient's progress in chart.
• Ensure that each patient receives appropriate care and treatment.
• Participates in discharge planning for patient

Qualifications:

• Current Physical Therapist License issued by the State Board of Physical Therapy (License must be active and unrestricted)
• Completion of an accredited physical therapy programs
• Reliable personal transportation
• Home Health experience preferred not required
• Newly graduates Welcome, We will guide you for this wonderful home health field.

Requirements:

• Active Physical Therapy License for the State of California
• Driving License
• CPR certification
• Professional liability insurance
