Job Title: Case Manager (Peer Support Worker)

Home Office:
Vienna, IL

Area of Focus:
Behavioral Health

Immediate Supervisor:
Community Resources Coordinator

Hours of Work:
Full-time, 40 hours/week

Minimum Qualifications:
High School Diploma/GED; lived experience is required.

Skills & Knowledge:
Have demonstrated skills in the field of services to adults or children, have demonstrated the ability to work with a team approach and accept supervision, and have demonstrated the ability to work constructively with Clients, treatment resources and the community. Additional knowledge of case management processes, area resources/service systems, group facilitation, curriculum planning, teaching of basic living skills, written and oral communications, crisis intervention/conflict resolution, word processing and basic computer skills are necessary. Knowledge of peer recovery tools and methods are preferred.

Special Requirements:
Travel to all agency sites when appropriate; participates in after hours and day call crisis rotation (if Bachelor’s degree or higher); transports clients to off-site locations using personal or agency vehicles; schedules evening or weekend hours as necessary; provide proof of a valid driver's license and personal vehicle liability insurance; at least 21 years of age; must complete required registry clearances and criminal fingerprint background checks; maintains CPR & First Aid training requirements.

Job duties include, but are not limited to:
  • Maintains compliance with mandatory trainings in accordance with agency policies and procedures, state and federal regulations, and CARF accreditation standards.
  • Provides crisis intervention services to stabilize consumer condition; links and refers to higher levels of care as needed.
  • Serves as a member of behavioral health treatment team that is primarily responsible for assisting individuals and families in improving wellness and autonomy through advocacy, support, education, identification of service resources, and service facilitation
  • Responsible for ensuring individuals have reduced barriers in accessing both internal and external health/behavioral health services by assisting with obtainment of resource needs
  • Provides comprehensive consumer care and case management services including access to transportation, state and federal entitlements, medical care, psychiatric care, housing, and other basic needs to support stabilization and achieve/maintain recovery.
  • Carries a client caseload as assigned.
  • Provides timely phone and in-person follow-up with individuals, families, and inpatient systems; assesses the efficacy of the developed safety plan and makes updates as needed.
  • Provides peer support services during crisis intervention when deemed necessary by QMHP/Crisis Director.
  • Completes direct service billing requirements in an efficient and timely manner.
  • Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
