Position Overview:

Engages with referred patients in both in-person and telephonic, motivational counseling sessions which are aimed at facilitating greater adherence to physician and/or registered nurse prescribed medical treatment as well as facilitating informed patient choices related to healthier lifestyles. Designs, with the patient, individualized action strategies and personal accountability structures which lead to greater control of the patient's disease (e.g. diabetes, high cholesterol, stress management, weight management, hypertension, asthma etc.) and more positive clinical outcomes. These sessions are designed to be aimed at achieving specific disease self-management goals or objectives. Independently manages a caseload of patients and provides disease management and coaching services. Tracks and monitor the progress of the patients over time and ensure progress toward a measurable goal.

The Perinatal Center is a part of the Women and Infants Service for Alta Bates Hospital. Candidates for this role will be working with pregnant moms in our care. Education provided to patients will include Baby Friendly Education. The person who fills this role will be working on-site in Oakland - this position is not WFH eligible.

Job Description:

EDUCATION:

  • Equivalent experience will be accepted in lieu of the required degree or diploma.
  • HS Diploma or equivalent education/experience


TYPICAL EXPERIENCE:

  • 1 year of recent relevant experience.


SKILLS AND KNOWLEDGE:

  • Ability to use appropriate communication skills in difficult situations, as well as the ability to document in electronic health record accurately and completely.
  • Spelling and grammar skills, including medical terminology and the ability to multi-task and prioritize heavy workload.
  • Knowledge of and demonstrate competence in proscribed educational content.
  • Knowledge of disease management and treatment protocols.

Pay Range 40.49 - 52.63

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
