Our healthcare organization is seeking a Patient Care Coordinator to join our team. The ideal candidate will be responsible for providing patient-centered care and coordinating services between patients, families, and healthcare providers. The successful candidate will have excellent communication and organizational skills, as well as the ability to multitask and prioritize tasks efficiently. The Patient Care Coordinator will also be responsible for scheduling appointments, maintaining patient records, and ensuring that all patient needs are met in a timely manner.

Responsibilities:

• Maintain patient records and ensure accuracy of information.
• Schedule patient appointments and coordinate with other departments to ensure timely care.
• Communicate with patients, families, and other healthcare providers to coordinate care.
• Assist in the development of treatment plans and follow-up care.

Job Type: Full-time

Pay: $17.00 - $27.00 per hour
